All lay employees may purchase coverage through Spectera Vision.
The plan is structured as a Point of Service (POS) plan, which gives members the choice of going “in” or “out” of network as follows:
In-network
- Yearly exams carry a $10 co pay;
- There is a materials co pay of $25 that applies to the entire purchase of eyeglasses (lenses and frames), or contacts in lieu of eyeglasses;
- Most frames are covered in full;
- Receive a $50 wholesale frame allowance at private practice providers, or a minimum $130 frame allowance at retail chain providers;
- Fitting/evaluation fees, contacts (including disposables), and up to two follow-up visits are covered-in-full for the most popular brands every year;
- Up to 8 boxes are included. $200 allowance is applied towards Toric, gas-permeable, and bifocal contact lenses; Refractive eye surgery is provided at a discount.
Out of network
- Yearly exams are covered up to $60;
- Single vision, bifocal and trifocal lenses are covered up to $40, $60, and $80 respectively; Frames are covered up to $45;
- Contact lenses are covered up to $200;
- The member must send itemized receipts to the Spectera in order to receive any reimbursements.
