As the Bishop of Metuchen, Bishop Checchio frequently celebrates the Mass and other liturgies through different parts of the Diocese. The Office of Divine Worship collaborates with Parishes and Institutions in preparing these liturgies.
When the Bishop is the principal celebrant at your Parish or Institution, please completed the appropriate downloadable PDF form below.

Episcopal Liturgical Preparation Form

When Bishop Checchio schedules to celebrate a liturgy at your Parish or Institution, please complete the Liturgical Preparation (LP) PDF Form which can be downloaded from the Downloadable Forms section.
If you require additional space for further details than the space provided on the PDF form, please provide the information in a separate word document when you submit the form.Please complete the LP Form at least two weeks prior to the scheduled liturgy and email it to worship@diometuchen.org. This allows adequate time for the Office of Divine Worship and the Office of the Bishop to review and follow up with the Parish or Institution as need be.
Please note that a submitted LP form is only for a single liturgy. For additional liturgies, please submit another separate form for each liturgy.

Please NoteFor those submitting liturgical details for a Confirmation at their parish, please use the Confirmation LP Form. This LP Form has information that is particular to this liturgy that is not asked for on the Episcopal LP Form.
